AWANA Registration

If you wish to order an AWANA uniform, please fill the lines below. First, specify which uniform your child needs from the list below.

• 3 & 4 year olds - Cubbies vest
• Kindergarten, 1st and 2nd grade - Sparks red vest (boy/girl)
• 3rd, 4th, 5th, and 6th grade - T&T Polo Shirt

Next, specify your child's size

• Cubbies - M, L, XL, XXL
• Sparks - M (8), L (10), XL (12), XXL (14), XXXL (16)
• T&T Polo Shirt - S, M, L, XL (adult size)

As Parent and/or Guardian, I do hereby authorize the treatment by a qualified and licensed medical doctor of the above minor(s) in the event of a medical emergency which, in the opinion of the attending physician, may endanger his/her life, cause disfigurement, physical impairment, or undue discomfort if delayed. This authority is granted only after a reasonable effort has been made to reach me. This release form is completed and signed of my own free will with the sole purpose of authorizing medical treatment under emergency circumstances in my absence for the Awana Club year, September 4, 2024 - May 7, 2025.

Please understand that an inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is a contagious disease that can lead to a potentially life-threatening illness. According to the Centers for Disease Control and Prevention, senior citizens and guests with underlying medical conditions are especially vulnerable. By attending our Awana program you voluntarily assume all risks related to exposure to COVID-19.
